Terms of Use

These Terms of Use govern the public website operated by Betula Mental Health PLLC under the Ingleside Health name. By using the website, you agree to these terms. If you do not agree, do not use the website.

General information only

Website content is general educational and service information. It is not medical advice, diagnosis, treatment, legal advice, claims assistance, or a substitute for advice from your own clinician, attorney, or VA-accredited representative.

No relationship through website use

Visiting the site, downloading a guide, sending an administrative email, or continuing to the Box-hosted intake does not by itself create a treating relationship, evaluator–evaluee relationship, attorney-client relationship, or engagement. A matter is accepted only through separate written engagement documents after eligibility, conflicts, scope, and payment arrangements are confirmed.

If a matter is accepted, a limited professional relationship may arise for the records review and any written opinion described in the signed engagement. It is not a treating relationship and does not include ongoing care, medication management, crisis care, or general medical advice.

Treatment and forensic-role conflicts

Betula Mental Health PLLC also provides treatment through a separate service line. A current or prior treatment relationship with Dr. Craig or Betula may create a professional conflict or compromise the independence of a forensic opinion. Ingleside screens these relationships before acceptance and may decline the evaluation or recommend a different independent evaluator. A person should not assume that treatment through Betula includes or entitles them to an Ingleside report, nexus opinion, DBQ, or other forensic work.

No emergency use

The website, administrative email, and Box-hosted intake are not continuously monitored and must not be used for emergencies. If you or someone else may be in immediate danger, call 911. Veterans and their loved ones may call 988 and press 1, text 838255, or use the Veterans Crisis Line chat.

VA independence and free assistance

Ingleside Health is independent and is not affiliated with or endorsed by the U.S. Department of Veterans Affairs or any state veterans agency. Ingleside Health does not prepare, present, prosecute, or file benefits claims and does not provide VA-accredited representation. Free assistance may be available from VA-accredited representatives and veterans service organizations.

The VA and other decision-makers control their own evidence standards and decisions. Medical opinions are offered only when clinically supportable. Nothing on the website promises that a matter will be accepted, that an opinion can be offered, or that any benefit or legal result will occur.

The full Illinois veterans-services disclosure displayed on VA-facing pages is incorporated into these Terms.

Eligibility and location

The website and services are intended for adults age 18 or older. Initial professional services are limited to eligible adults physically located in Illinois or Texas when the service is provided. Availability also depends on licensure, conflicts, capacity, records, requested scope, and applicable law.

Public website and Box-hosted intake

The public website does not operate an inquiry or intake form. When available, `/intake` provides a neutral link to a Box-hosted Form operated by Betula. Medical records and intake answers are submitted to Box, not to this website or Vercel.

Do not send medical records, diagnoses, Social Security numbers, VA claim numbers, or sensitive health information through ordinary email or place them in a website URL. Use only the Box-hosted intake for requested information. You represent that information you submit is accurate to the best of your knowledge and that you are authorized to provide it. Do not submit another person's records without authority.

Acceptable use

You may not misuse the website; attempt unauthorized access; defeat security controls; interfere with availability; introduce malicious code; scrape or copy the service in violation of law; impersonate another person; submit unlawful, fraudulent, or infringing material; or use the site to harm another person.

Pricing and payment

Public pricing is general information. The signed engagement for an accepted matter controls the contractual scope, when fees are earned, cancellation and refund terms, and additional services. The Good Faith Estimate identifies expected charges but is not a contract. No fee is contingent on a VA or legal outcome.

The $500 initial physician records-review fee is non-refundable if you cancel after Dr. Craig begins substantive physician review, even if the review is not yet complete. Administrative screening, file receipt, scheduling, and payment alone do not count as substantive physician review. If you cancel before that review begins, or Ingleside declines or cannot provide the agreed initial review, we refund that fee. A completed review that does not support the requested opinion is still a performed review and does not qualify for a refund on that basis. If an opinion is clinically supportable and you choose to proceed, the remaining $1,000 is due before final work is completed. Refunds required by law remain available. These terms apply to new engagements and do not replace an existing matter's signed fee terms.

Ingleside services are self-pay. Ingleside does not submit claims to an insurer and does not promise that an insurer, health plan, FSA, HSA, or other payer will cover or reimburse any charge. If Ingleside provides a receipt or superbill, that document is not a coverage determination or reimbursement guarantee.

Referrals and financial independence

Ingleside does not pay or accept referral fees, kickbacks, commissions, a percentage of professional fees, reciprocal patient steering, or preferred-placement consideration for an evaluee referral. A referring attorney, accredited representative, VSO, directory, marketer, or other third party does not control whether a matter is accepted, the fee, Dr. Craig's method, or any clinical conclusion. Any lawful fixed-fee administrative or marketing arrangement must be documented at fair market value, unrelated to the volume or value of referrals, and reviewed by counsel before use.

Reviews and testimonials

No review or testimonial is a promise or evidence of the result another person will receive. Ingleside will not buy or condition an incentive on positive sentiment, write or procure fake reviews, suppress negative reviews, or publish a testimonial that discloses health information without a specific written authorization. Material relationships and incentives must be clearly disclosed. Any outcome-related statement must be truthful, representative, and substantiated; a general “results may vary” disclaimer is not enough to cure a misleading claim.

Intellectual property

Website content is owned by Betula Mental Health PLLC or used with permission. You may view and make a personal copy of public educational content for lawful, non-commercial use. You may not remove notices, falsely attribute content, or reproduce, sell, or commercially exploit content without permission or a legal right.

You retain rights in information you submit. You give Betula only the limited rights necessary to receive, secure, review, and use the information for the requested service, legal compliance, and ordinary operations. Medical records and evaluation materials are not licensed for unrelated advertising or model training.

Third-party sites

Links to government, licensing, professional, payment, or other third-party services are provided for convenience or verification. Betula does not control those services. A link does not imply endorsement, and third-party terms and privacy notices govern their services.

Website availability and accuracy

We work to keep public information accurate and the website available, but educational content may not address every fact, jurisdiction, or later legal change. The website is provided on an “as available” basis to the extent permitted by law. We may correct, change, suspend, or discontinue public content prospectively.

Nothing in these Terms excludes or limits liability, duties, or rights that cannot lawfully be excluded, including applicable professional duties, privacy rights, or liability for intentional misconduct or professional negligence.

Privacy

The Website Privacy Policy governs public-site information. The Health Information Privacy Notice and signed engagement documents govern health information and accepted matters. This notice is not a HIPAA Notice of Privacy Practices. If HIPAA applies to a service, Betula will provide and post the applicable Notice of Privacy Practices as required by law.

Changes

We may update these Terms prospectively by posting a revised version and effective date. The signed engagement controls contractual terms. Authorizations govern permitted disclosures. Privacy notices describe information practices and rights. A Good Faith Estimate describes expected charges and is not a contract.
