Health Information Privacy Notice

Who we are

Betula Mental Health PLLC operates an independent physician records-review and written-opinion service under the Ingleside Health name. This notice applies to that service and inglesidehealth.com. References to “Betula,” “Ingleside,” “we,” and “us” in this notice mean Betula Mental Health PLLC and its authorized workforce when acting for the Ingleside service.

Ingleside does not provide ongoing treatment, emergency services, legal representation, or claims filing. Submitting information does not by itself create a professional relationship. Any professional relationship is limited to the accepted records review, opinion, or separately agreed evaluation described in the written engagement.

Why you are receiving this notice

This notice explains how we collect, use, protect, retain, and disclose personal and health information and how you may make a privacy or records request. We use safeguards appropriate for sensitive health information. Illinois and Texas medical-record, mental-health-confidentiality, consumer-protection, and breach-notification laws may also apply.

This notice is not a HIPAA Notice of Privacy Practices. If HIPAA applies to a service, Betula will provide and post the applicable Notice of Privacy Practices as required by law.

Information we collect

Public website and administrative email

The Ingleside public website does not operate an inquiry or intake form. Vercel may process ordinary technical information needed to deliver and protect inglesidehealth.com, such as IP address, date and time, requested page, browser/device information, and security events. If you send an administrative email, the email systems receive the address and contents you choose to send.

Do not put diagnoses, symptoms, medical history, records, full dates of birth, claim numbers, or other sensitive details in ordinary email or a website URL. Use the Box-hosted intake for any requested information.

Box-hosted intake and professional records

If you continue from inglesidehealth.com, the website sends you to a Box-hosted intake operated by Betula. Medical records and intake answers are submitted to Box, not to the public website or Vercel. Depending on the accepted service, Betula may receive identity and contact information; location and eligibility information; medical, mental-health, service, and benefit records; authorizations and consents; information about the requested evaluation; payment and transaction records; communications; and the professional report or work product.

We ask for information reasonably needed to screen, accept, perform, document, and deliver the requested service. Please do not submit another person's records unless you are the person, a verified personal representative, or otherwise legally authorized.

How we use information

We may use information to:

  • respond to an administrative question or a Box-hosted intake submission;
  • determine whether we can accept a matter, including location, licensure, identity, scope, and conflict checks;
  • review records, conduct an accepted evaluation, exercise professional medical judgment, and prepare or correct a report;
  • communicate with you and deliver approved documents;
  • provide an estimate, collect payment, issue a receipt, and maintain business records;
  • maintain quality, security, access control, audit, training, legal, accounting, and compliance functions;
  • prevent fraud, abuse, or security incidents;
  • respond to a safety concern or emergency as permitted by law; and
  • comply with a valid legal obligation.

We do not use health information to guarantee, manipulate, or predetermine a medical opinion or government outcome.

When we disclose information

We disclose health information only as authorized, permitted, or required by applicable federal and state confidentiality law.

We may disclose information:

  • to you or a verified personal representative;
  • to the attorney, accredited representative, government agency, clinician, or other recipient you specifically authorize;
  • to Box and another service provider that needs limited access to operate the service and is subject to appropriate confidentiality, security, and contractual restrictions;
  • to a payment processor for the minimum information needed to process or document payment;
  • to legal, accounting, security, or regulatory advisers under appropriate duties;
  • when required by a valid court order, administrative order, subpoena, reporting law, or other legal process after applicable protections are considered, including any additional Illinois mental-health-record requirements; or
  • when permitted by law to prevent or address a serious and imminent threat.

Payment by an attorney, family member, organization, or other third party does not by itself authorize that payer to receive medical information or control the evaluator's opinion.

An independent report is created for the purpose stated in the engagement. If you authorize a report for use in a VA claim, immigration matter, lawsuit, or another proceeding, it may be filed, quoted, challenged, or placed in an agency or court record. Information lawfully disclosed to a recipient that is not governed by the same privacy law may be redisclosed by that recipient.

We do not sell health information. We do not disclose clinical records or intake answers to advertising platforms, data brokers, or public-site analytics providers, and we do not use them to create advertising audiences.

Our service providers and systems

Clinical intake and records are maintained in Betula's Box enterprise. Betula has a signed Business Associate Addendum with Box and uses an Enterprise Advanced account. The public Vercel website supplies a neutral outbound link; it does not host, embed, proxy, or receive the answers or files submitted in Box.

Other providers may process narrowly limited information for website hosting, security, communications, payment, legal, or accounting functions. We do not authorize those providers to use health information for their own advertising.

Your privacy and records choices

Subject to identity verification, applicable law, the nature of the record, and lawful exceptions, you may ask us to:

  • provide access to or a copy of information maintained about you;
  • correct objective information that is inaccurate or incomplete;
  • communicate with you through a particular channel or at a particular address;
  • restrict a use or disclosure;
  • identify certain disclosures;
  • provide a copy of this notice;
  • recognize or update a verified personal representative;
  • revoke a prior authorization for future disclosures; or
  • delete information when no professional, legal, safety, accounting, litigation-hold, or record-retention duty requires us to keep it.

A correction request does not require Dr. Laura Craig to change an independent professional judgment. We may correct objective errors while preserving the original record, the request, and the professional conclusion.

Illinois and Texas law may require a qualifying copy of existing medical records to be provided without charge when requested for a federal veterans' disability-benefits matter and the statutory conditions are met. That record-copy rule does not make preparation of a new evaluation, opinion, addendum, or testimony free.

How to request your record

Email the Betula/Ingleside Privacy Coordinator at daniel@inglesidehealth.com with the subject “Private records request.” Do not attach medical records to ordinary email. State your name, a safe way to contact you, the service involved, the record or action requested, the desired format, and whether another person is authorized to act for you. We will use a secure method to verify identity and obtain any needed authorization or supporting document.

For a Texas veterans' disability-benefits copy request, include the agency statement or other documentation required by Texas law to confirm the benefit application or appeal. We will explain any lawful denial, fee, extension, or additional verification requirement.

Security and retention

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including restricted accounts, private clinical folders, platform authentication, encryption supplied by contracted platforms, and access logging. No electronic system is risk-free, and you should not send medical records by ordinary email.

We retain professional and business records for the period required or permitted by applicable medical-record, licensing, tax, limitation, litigation-hold, and other law. When retention is no longer required, we delete, destroy, or de-identify information through the applicable system process. A deletion request cannot override a duty to preserve a medical or forensic record.

If a reportable breach or security incident affects your information, we will investigate and provide notice as required by applicable law.

Texas contacts and complaints

Texas consumers may use the records-request process above. Questions or complaints about a Texas physician may be directed to the Texas Medical Board. The Board's required English and Spanish complaint notice is available on our Texas Medical Board Complaints page.

Texas consumers may also file a consumer complaint with the Texas Attorney General. We will not retaliate against you for making a good-faith privacy or regulatory complaint.

Illinois contacts and complaints

Questions or complaints concerning an Illinois physician license may be directed to the Illinois Department of Financial and Professional Regulation. Privacy and records questions should first be sent to the Privacy Coordinator at daniel@inglesidehealth.com so we can investigate and respond. We will not retaliate for a good-faith complaint.

Emergencies

Neither the public website nor Ingleside intake is monitored for emergencies. If you or someone else may be in immediate danger, call 911. Veterans and their loved ones may call 988 and press 1, text 838255, or use the Veterans Crisis Line chat.

Changes and contact

We may update this notice when our services, systems, or legal obligations change. The current version and effective date will remain posted on inglesidehealth.com. Material changes will apply as permitted by law and any applicable agreement.
