Betula Mental Health PLLC operates the Ingleside Health service and inglesidehealth.com. This Website Privacy Policy explains how we handle information associated with the public website.
Clinical and evaluation records are governed by our Health Information Privacy Notice, applicable state confidentiality law, and signed engagement documents. If HIPAA applies to a service, Betula will provide and post the applicable Notice of Privacy Practices as required by law.
Information we collect
Information you provide
The public website does not operate an inquiry or intake form. If you choose the Box intake link, you leave the public website and submit information directly to Betula's Box-hosted Form. If you email the administrative address, do not include diagnoses, symptoms, records, claim numbers, Social Security numbers, or other health details; Betula will direct any needed information to the Box-hosted process.
Information collected automatically
Our hosting and security providers may process limited technical information needed to deliver and protect the website, such as IP address, device/browser type, timestamps, requested pages, and security events.
If enabled, we use limited, cookie-free website analytics to understand aggregate page usage. Analytics is disabled on `/intake` and is not allowed to receive Box Form contents, uploaded records, diagnoses, claim details, or intake data. We may store limited attribution information in your browser, such as the first landing page, referring domain, and campaign parameters. We do not place public-site attribution information in the clinical record.
Separate access-controlled review tools may process authorized reviewer identifiers and feedback about draft forms or synthetic demonstrations. Those tools are not patient intake channels and may not be used for veteran names, claim numbers, medical records, diagnoses tied to a person, or other patient or claim information.
How we use information
We use public-website information to:
- operate, secure, troubleshoot, and improve the website;
- prevent abuse, fraud, and unauthorized access;
- understand aggregate website usage and referral sources; and
- meet legal, regulatory, and professional obligations.
We do not use medical information for advertising audiences or cross-context behavioral advertising.
When we disclose information
We may disclose limited website information to:
- Vercel, which hosts and protects the website and may provide limited analytics;
- Box, when you follow the separate Box intake link; Box receives the intake information directly under the Health Information Privacy Notice;
- a payment provider, if you follow a separate payment link; the provider receives only the minimum transaction information needed for payment;
- professional advisers and service providers bound by appropriate confidentiality and data-protection obligations; and
- government authorities or other recipients when required by law, necessary to protect rights or safety, or authorized by you.
We do not sell personal information. We do not sell consumer health data. We do not share personal information for cross-context behavioral advertising.
Clinical information and Box-hosted intake
Clinical records and intake responses are submitted directly to Betula's Box Enterprise Advanced environment. They are not submitted to or returned to Vercel. Access is limited according to role, and a signed Business Associate Addendum is in place with Box. A payment provider may process narrowly limited transaction information but must not receive clinical narratives or records.
Please do not email medical records. Use the Box-hosted intake for any information or records Ingleside requests.
Retention
We retain professional, health, business, and website-security records for the periods required or permitted by applicable medical-record, licensing, tax, limitation, legal-hold, and other law. The period varies by record type, purpose, and state. When no professional or legal duty requires continued retention, we delete, destroy, or de-identify the information through the applicable system process.
Security
We use administrative, technical, and physical safeguards designed to protect information. These include access controls, encryption in transit, secure cloud storage, audit records, and workforce confidentiality obligations. No system can guarantee absolute security.
Your rights and choices
Depending on the information and law that applies, you may ask to access, correct, receive, restrict, or delete information, withdraw a consent, request confidential communications, or obtain information about certain disclosures. Rights involving health information are described in the Health Information Privacy Notice. If HIPAA applies to a service, Betula will provide and post the applicable Notice of Privacy Practices as required by law.
To make a request, use the privacy contact listed below. We may verify your identity and authority before acting. We will not discriminate or retaliate because you exercise a privacy right.
You may use an authorized agent where applicable. We may ask for proof of the agent's authority and may verify your identity directly. If we deny an appealable state-law request, we will explain how to appeal.
Children
The website and services are intended for adults age 18 or older. We do not knowingly collect personal or health information from a minor through the website or intake. If you believe a minor has submitted information, contact us so we can assess and take appropriate action.
External links
The website links to government, licensing, professional, payment, and other third-party websites. Their privacy practices are governed by their own notices. A link does not imply endorsement unless expressly stated.
Changes to this policy
We may update this policy prospectively. We will post the revised policy and effective date. If law requires additional notice or consent for a material change, we will provide it before the change applies.
